subjects provided informed consent. The final NESARC sample consisted of 43,093 persons; overall raw response rate was 81%. The final NLAES sample consisted of 42,862 persons with a response rate of 90%. The analytical samples excluded those with missing height or weight, or incomplete or indeterminate family history information, pregnant women, and underweight individuals (BMI <= 18.5 kg/m2). This resulted in N=39,312 for NLAES and N=39,625 for NESARC, or 91.7 and 92.0% of the total samples, respectively. Further details for both surveys, and comparative descriptions of methods are available elsewhere.21–24
