Height and weight were measured using a standard protocol, and body mass index was calculated as weight/height2 (kg/m2). Blood pressure was measured at the end of the examination after a five minute rest in the sitting position. Measurements were performed with an automatic device (Spengler SP9; Spengler, Asnières sur Seine, France), which also recorded heart rate. A standard cuff size was used, but a larger cuff was available when necessary. A standard 12 lead electrocardiogram was recorded.
