disease excluding myocardial infarction; fatal and non-fatal stroke (including ischaemic, haemorrhagic, subarachnoid, and unclassified subtypes of stroke); fatal and non-fatal heart failure; and mortality from other cardiovascular causes, including cardiac dysrhythmia, hypertensive disease, sudden death, and aortic aneurysm.7, 17, 25 In analyses of cardiovascular disease subtypes, participants contributed follow-up time until the first outcome recorded (ie, cardiovascular deaths preceded by non-fatal outcomes were not included). Event times were censored at the end of follow-up or death from non-cardiovascular causes.
