The NIS is an annual, random-digit-dialing telephone survey of approximately 34,000 US households with at least one child 19–35 mo old [30]. After answering questions about the resident child's immunization status, participants are asked for approval to contact providers to obtain the child's immnunization records. The NIS neither conducts public outreach nor provides financial incentives.
