Height (m) and weight (kg) were measured with a clinical calibrated scale. Body Mass Index (BMI = kg/m2) was computed from those measures, and individuals were categorized as either normal (0, BMI < 25; n = 1292, 55%), overweight (1, BMI = 25–30; n = 838, 36%), or obese (2, BMI > 30; n = 222, 9%). The median date of visit was 1979, which in part explains the relatively low prevalence of obesity in this American sample.
