A composite measure was calculated by taking the prorated sum of the items for participants who responded to at least three items. Response options were 0 (no) and 1 (yes) for binary items. For other questions, the response was made using a 4-point ordinal scale. Items were rescaled to range between 0 and 1 (0, 0.33, 0.66, 1) so that ordinal and binary items were weighted similarly into the prorated sum.
