Sixty-three adolescents made a suicide attempt during the 1-year of follow-up based on either adolescent or parent report. Maternal and paternal history of mental health problems, higher SIQ-JR, CAFAS, hopelessness (BHS), CDRS-R, and internalizing symptom (YSR Internalizing) scores were individually associated with increased risk of attempt during follow-up. The unadjusted hazard ratios are as follows: 2.21 for maternal history of mental health problems, 95% CI: 1.30–3.75, p = .003; 2.16 for paternal history of mental health problems, 95% CI: 1.16–4.03, p = .02; 1.30 for a 10-point increase in SIQ-JR, 95% CI: 1.14–1.48, p <.001; 1.26 for a 20-point increase in CA-FAS, 95% CI: 1.12–1.42, p < .001; 1.51 for a 5-point increase in BHS, 95% CI: 1.22 –1.87, p < .001; 1.29 for a 10-point increase in CDRS-R, 95% CI: 1.10–1.52, p = .002; 1.18 for a 5-point increase in YSR Internalizing, 95% CI: 1.05–1.32, p = .004. Univariate analysis indicated that adolescents who had histories of multiple suicide attempts were not significantly more likely to make an attempt during follow-up (unadjusted HR = 1.59, 95% CI: 0.95–2.64, p
