The presence or absence of one or more suicide attempts during each follow-up period was coded as a dichotomous indicator, using suicidality items from the Mood Disorders module of the NIMH DISC-IV (DSM-IV criteria). A suicide attempt was indicated for a positive response to “Have you ever, in your whole life, tried to kill yourself or made a suicide attempt?” The time frame was adapted to capture data between assessments. If a suicide attempt was indicated over the course of the study by either adolescent or parent report, the month during which the event occurred was recorded (if there was a difference between the adolescent and parent report, the earlier of the 2 months was used).
