Nearly all (699 out of 706, i.e., 99%) interviewees agreed to be contacted for an additional follow-up evaluation. As anticipated (Hypothesis 2), the likely participation rate among located living subjects was high (proportion and 95% confidence interval = 0.89 [0.86, 0.91]) for a subsequent follow-up study.
