race, education, smoking, history of diabetes, BMI, caloric intake, and physical activity. Exploratory analyses adjusting for history of pancreatitis also were conducted, but results are not presented as risk estimates did not differ by at least 10%. Odds ratios and 95% CIs are not presented in tables when the number of observations was five or fewer. All statistical tests were two-sided and considered statistically significant when p < 0.05. Statistical analyses were conducted using SAS software V9.1 (SAS Institute, Inc., Cary, NC) and results are presented for age-adjusted and final multivariable models.
