The 15-item self-report Suicidal Ideation Questionnaire—Jr. (SIQ-JR) assessed severity of adolescents’ suicidal thoughts. It has demonstrated predictive validity for psychiatrically hospitalized adolescents (King et al. 1997), and internal consistency in this sample was high (.92). The SIQ-JR total score ranges from 0 to 90 at 1-point increments. Sample items include: “I thought it would be better if I was not alive” and “I thought about writing a will.”
