The protocol was approved by the institutional review board at each site, and written informed consent was obtained from all subjects. Of the subjects studied in the present report, 85.7% were first-degree relatives of the proband (father, mother, sibling, offspring). The sample was 80.0% White, 12.5% African American, 4.5% Hispanic, and 3.0% was classified as Native American, Pacific Islanders and “other.”
